TRISTAR Wellness works in collaboration with each employer to introduce the program and educate employees. It starts with a step-by- step approach that can be tailored to any employer's needs.

- Employees participate in an initial Wellness assessment
- Initial Wellness Testing: Assessment score and custom health improvement plan for the employee
- The employee receives an individualized plan of action within 2 weeks of submitting their assessment form, which provides the employee with their wellness score, recommendations to improve the score, and the date for the next assessment.

Disease Care Management
Disease Care management is for employees who need more help than those at the healthy end of the scoring spectrum. Disease Care Management includes case management, utilization management, and disease management. Care management can help at-risk employees achieve better health through effective patient management. Effective patient management helps them continue with the behaviors and clinical approaches that have been proven to work.
Total Health View
We look at the employee's complete health – not just a single condition, but the overall health status, to find the right combination of information and support to help them achieve their optimal health status. If it is unreasonably difficult due to a medical condition for an employee to achieve the standards for the reward under this program, the Wellness Nurse will work with the employee directly to create a customized program.

Employees' work with TRISTAR Medical Case Managers to better understand their physical conditions, get their questions answered, and learn about ways to improve their health while integrating healthier lifestyles into their daily routine.
